You have three convenient ways to order:

1)  To order online, visit the MVA website.

2) Visit any full-service MVA location and fill out an application for the Bay plate in person. Click here to find the nearest location or call 800.950.1682 for location information.

3) You can also download the application and then mail it to the MVA.
